Insurance & Scheduling
We are in-network with select insurance plans through Headway and accept certain out-of-network benefits. If applicable, we can provide courtesy superbills and assist with out-of-network reimbursement.
Once you submit the contact form or leave a message, we’ll get back to you within one to two business days to discuss next steps.

Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You Have The Right
You have the right to receive a Good Faith Estimate for the total expected cost of non-emergency services, including medical tests, prescription drugs, equipment, and hospital fees.
Get an Estimate
Request a written Good Faith Estimate from your health care provider at least 1 business day before your medical service or item. You can also ask any provider for a Good Faith Estimate before scheduling.
You Can Dispute
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Save a Copy
Save a copy or picture of your Good Faith Estimate. For questions or more information, visit www.cms.gov/nosurprises or call 800-985-3059.
